Simply select any provider at any location. Enrollment is quick and easy and you can use your benefits immediately.

One low annual fee includes no-charge preventive dental visits plus 15% off all other services.

The patient must not be enrolled in any dental insurance program. If at any time during the rolling enrollment year the patient becomes insured, the remaining benefits are not applicable. Dental services must be paid at time services are rendered to be eligible for 15% discount.
